This form is to sign up for the dinner at the Global Missions Conference on Tuesday, August 25th at 5:15pm.
On Tuesday at the Global Missions Conference, we will have two different options for meals: pizza or a main entree. To help us order the right amount of each, please indicate how much of each your crew will eat. Then, make a payment that adds the pizza number and the main entree number together.
For instance, if your two kids want pizza, mark "2" under the pizza option. If you and your spouse will eat the main entree, mark "2" under the main entree option. Then, pay for "four or more people" in the payment line. Thanks!
